You can achieve above by following below:
Create a list with the fields mentioned in the snapshot
Modify the form with Nintex, InfoPath or Client side rendering depending upon availability & comfort in mentioned options
For sub-site creation, you can either JS CSOM, web-services etc.

One done, you can ask user to request site using newform.aspx of the list

I have SharePoint Online enabled for my Dynamics CRM Online. I have a documents subgrid showing on my account form. Everytime a document is uploaded to Sharepoint from within CRM, an email should be fired to the owner of the record.
I tried using this with Flow but it is difficult as the SharePoint Folder structure is as follows: Account > AccountI_GUID
AccountII_GUID
and so on
I could have alerts in SharePoint enabled. However, I would have to do this each time a new account is created in CRM. There is 13000 accounts in CRM already. So, I would have to go into each of the folders and enable alerts.
Would you have any other solutions?
I was thinking of maybe getting a count of all the documents in SP and showing the count in CRM. If the count has changed then trigger a workflow. But I am not sure how to go about that. Please help.
I would probably go about this by creating a Powershell script to iterate through all of the account document libraries to set up an alert on each one. You could then run this at the end of each day to pick up any new accounts.
After a quick google I came up with this which may be of use Create Alerts using CSOM
I know there are also ways in Dyanamics to remove the GUID from the library but I'm no Dynamics expert so not sure what functionality this may effect.
I have created a list of around 443 items
I have set item limit to 1 so that it shows one item at a time

I need to know how do i show the random items in sharepoint list and how to create a column whic user will fill and submit the answers.
Then sharepoint column will check using that column whether the answer is right or not
Survey may not suit your case.
Because the rating/grading functionality would be very difficult to address with Surveys. I'd also recommend you to create ASP.NET pages, but create them as application pages.

After creating this page just deploy to _LAYOUTs and call this page into your web part page. I've a different thought of using PageViewer web part. Do not use page viewer webpart for this kind of quiz functionality. If you have a single plain vannila page, you can go for page viewer web part. Quiz would be a full-blown ASP.NET web application. Its' better to deploy that as application page instead of a page viewer web part approach.

I need suggestion on share point workflow development.
Currently I am working on Share Point 2013. I have root site and its sub-sites.
Following is the challenge:
1. I have to move document from "Document Library" of root site to one of its sub-site using share point workflow.
2. Move document from sub-site to sub-site of root site.
I am not sure whether I should use "List workflow" or "Site workflow" as I am new in Share point.
I am using share point designer 2013 to develop workflow.
I search a lot on google but no luck on this.
Please provide yours valuable suggestion.

No, but there is a workaround. Right click on the tables you don't want to be visible, and select the option to hide them individually. You can also rename the table-views in that pane, so they appear to have more user-friendly names for your end-users.

The best way to achieve something like this would be using a hyperlink control attached to a calculated column in your table/query where the URL and display text are computed and linked together automatically. If you need list functionality, you can allow the user to select an item in the list, and have that automatically update a separate hyperlink control to provide the link functionality.

Access 2013 Web Apps cannot pull external data at this time. Access 2013 Web Apps only accept incoming data connections e.g. a MS-SQL or oData service can use industry-standard methods to connect to your Access Web App and validate data.

The only file support currently provided by Access Web Apps is in the form of images uploaded to an image field inside a table. This is useful to add people's photos to their records in a "contacts" table, and things like this. Other than that, you will need to use a custom SharePoint library interface for this type of functionality.

This functionality is handled within SharePoint, not within Access or SQL Azure. Secure your SharePoint site collection as needed, and these security settings will apply appropriately to read and/or write operations by SharePoint users within your database. e.g. a SharePoint "visitor" (read-only access) will not be able to edit any data in your database - only view it.

In Access 2013 Web Apps, VBA is no longer supported. Macros have to be written using the GUI "wizardy" macro interface, which greatly limits the scope of available operations. As a result, you cannot perform more complex tasks, such as making calls to external web-services. However, for basic data operations and UI operations, the GUI macro interface serves the purpose well.

The URL is randomly assigned when you create an Access 2013 Web App, but is guaranteed to never change after it is assigned. Therefore, you could reasonably use your DNS/web-host to "mask" the URL with a CNAME or other type of HTTP redirection. We got a standard-webpart, which displays the document library using views. We can use these views using the SharePoint Designer to customize to our customers needs.
Now the customer wants to have filter/search - option that is used on the title of the documents.
What would be the best ways without writing custom code?
Can it even be done just using SharePoint-Designer and standard-webparts?
